Abstract
Introduction & Objectives
Intravesical bacillus Calmette-Guerin (BCG) therapy is an effective and widely used treatment for non-muscle-invasive urothelial carcinoma of the bladder. Local complications are frequent whereas systemic complications are rare but can be serious, and their management is not well known.
Materials & Methods
We describe retrospectively the records of 32 patients who had been diagnosed with non-muscle-invasive urothelial carcinoma of the bladder and treated by transurethral resection of the bladder and intravesical instillation of BCG during 2014, 2015 and 2016.
Results
All patients were male, with a median age of 73 years (range 57–80). All of them were men. 21.8% were carcinoma In situ, 6.3% were T1 low-grade, 3.1% were Ta and 68.8% were T1 high-grade.
73% of our patients were treated with induction BCG instillations (six-weekly schedule) and maintenance schedule (one-monthly schedule), and 27% of them without maintentance instillations. However, 15.7% of them stopped maintentance schedule due to local toxicity (58%) and due to unknown refusal (42%). One of them stopped because of developping tuberculosis. No other cases of systemic toxicity has occurred.
The most frequent local symptoms were dysuria (71.9%), macroscopic haematuria (31.3%) , increased urinary frequency (28.12%), cystitis (15.6%) ,bladder pain (12.5%), and incontinence (9.4%).
Conclusions
Majority of patients do benefit from BCG, nevertheless complications following intravesical BCG therapy can ocurred. Local complications are frequent but mild. However, severe complications can ocurred in spite of the fact that they are rare.
